Rear View Mirror

My friend's rear view mirror glass was damaged due to an accident. Where can she find a universal rear view mirror? Can it be mounted onto the current one or does it have to be removed to be replaced??

Finally what is the price of a universal rear view mirror?

Is the rear view mirror mounted to the windshield?

if so a replacement can be purchases which will fit on the original mount and be just like new. I can provide more info if needed.
